Waiver for Celebrate You Workshop

Waiver and Release of Liability:

I, the undersigned, am the legal guardian of the minor named above (hereafter referred to as "Participant"). I understand and acknowledge that participation in the celebrate youworkshop involves activities that may include, but are not limited to, meditation, breathing exercises, and various forms of movement and relaxation techniques.

I acknowledge that there are inherent risks associated with such activities, and I hereby voluntarily assume all risks associated with my child's participation in the workshop.

I agree to the following:

1. Assumption of Risk: I understand that participation in the celebrate you workshop involves potential risks and I voluntarily assume full responsibility for any risk of loss, property damage, or personal injury that may result from participation.

2. Release of Liability: I hereby release, waive, discharge, and covenant not to sue the workshop facilitators, organizers, and venue owners (collectively referred to as "Releasees") from any and all liability, claims, demands, or causes of action that may arise from Participant’s participation in the workshop, whether caused by the negligence of the Releasees or otherwise.

3. Medical Authorization: In the event of a medical emergency, I authorize the Releasees to seek medical treatment for the Participant. I agree to bear any costs associated with such treatment.

4. Behavioral Expectations: I understand that the Participant is expected to behave respectfully and follow the instructions provided by the workshop facilitators. I acknowledge that disruptive behavior may result in the Participant being removed from the workshop.

5. Photographs and Media Release: I grant permission for the Participant’s image and likeness to be photographed, filmed, or otherwise recorded during the workshop. These materials may be used for promotional purposes by the workshop organizers without compensation.

6. Agreement to Terms: I have read and fully understand this waiver and release of liability. I certify that I am the legal guardian of the Participant and that I have the authority to sign this agreement on behalf of the Participant.
